Job summary

Continuum Rehab Group in Rockwall, Texas is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing and Long Term Care facilities.

You'll provide evidence-based interventions under the supervision of a Physical Therapist, develop rehab programs, monitor progress, and communicate care plans to families and staff. Qualifications

• Licensed Physical Therapy Assistant required.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides consultation and counseling to patients and families, caregivers and other service providers.
• Assists with discharge planning and documentation of services provided.
