Job summary

Elara Caring is seeking a Physical Therapist Assistant for Home Health in McHenry, IL. You will provide PT services per the plan, help patients reach mobility goals, and educate families on home therapy techniques. Must hold a PTA license and have completed an accredited PTA program.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• Graduate of an accredited PTA program.
• 6+ months experience as a Physical Therapist Assistant preferred.

Responsibilities

• Provide physical therapy services according to the treatment plan established by the Physical Therapist.
• Help patients work toward therapy goals that improve mobility, function, and independence.
• Monitor patient progress and report changes to supervising PT.
• Educate patients and caregivers on home therapy programs and techniques.
• Teach patients and families how to safely use assistive devices (canes, walkers, wheelchairs, prosthetics, orthotics).
• Deliver safe, high-quality therapy in accordance with physician orders and state guidelines.
