Job summary

Elara Caring is seeking a Physical Therapist Assistant to join our home-based care team in Illinois. You will deliver direct PTA services under guidance, following the Plan of Care and physician orders to ensure quality and continuity of care.

Travel up to 50% is required within the branch/office coverage area. A PTA license, graduation from an accredited PTA program, and reliable transportation are essential for this role.

Qualifications

• Graduate of an accredited PTA program.
• Current, unrestricted PTA license for the state of work.
• 6+ months experience as a PTA preferred.
• Willingness to travel 50% within branch/office coverage area.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide direct PTA services under supervision to ensure quality and continuity of care.
• Follow Plan of Care established by Physical Therapist and report changes promptly.
• Adhere to State Practice Act Guidelines and facility policies.
• Instruct patients and families in proper use of prosthetic, orthotic, and assistive devices.
• Manage patient transfers and assist with positioning and mobility as required.
