Job summary

ABL Health Care in Gilroy is seeking a dedicated Physical Therapist Assistant (PTA) for full-time home health services. You will evaluate patients, develop and implement individualized treatment plans, and help improve mobility and quality of life while collaborating with the Clinical Director and team.

This role supports patients in their homes within a 15-25 mile radius, requires CA PTA license, 1 year Home Health experience, and a valid CA driver’s license.

Qualifications

• Current Physical Therapist Assistant (PTA) License to practice in California - Required.
• Experience in Home Health care - 1 Year Required.
• Current CA Driver’s License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
