Job summary

ABL Health Care in Livermore is seeking a dedicated, full‑time Physical Therapist Assistant (PTA) to join our Home Health team. You will evaluate patients, develop treatment plans, and help improve mobility and quality of life while collaborating with our Clinical Director and staff in the field.

This role requires an active California PTA license, at least 1 year in home health, a valid CA driver's license, and willingness to travel 15–25 miles regularly.

Qualifications

• Current Physical Therapist Assistant (PTA) license to practice in California.
• Experience in Home Health care – 1 year required.
• Current CA Driver’s License.
• Able to travel within a 15–25 mile radius on a regular basis.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.
