Job summary

ABL Health Care in Milpitas, CA is seeking a full-time Physical Therapist Assistant (PTA) for Home Health. The PTA will evaluate patients, develop individualized treatment plans, and work to improve mobility and quality of life.

Qualifications

• Current Physical Therapist Assistant license in California.
• Minimum 1 year of Home Health care experience.
• Valid CA Driver’s License and ability to travel within a 15–25 mile radius.

Responsibilities

• Provide physical therapy services per the patient’s plan in coordination with the Interdisciplinary Team.
• Monitor patient conditions, administer treatments, and report changes to the appropriate stakeholders.
• Maintain accurate clinical records using company devices and submit according to policy.
• Adhere to all company policies to ensure high standards of care.
