ABL Health Care in Walnut Creek, CA is seeking a full-time Physical Therapist Assistant (PTA) to join our Home Health team. You will evaluate patients, develop treatment plans, and help improve mobility and quality of life, working with the Clinical Director and staff both in the field and remotely as needed.

Requires CA PTA license, 1 year home health experience, valid CA driver’s license, and willingness to travel within a 15–25 mile radius.

Qualifications

• Requires California PTA license to practice in CA.
• At least 1 year of Home Health care experience.
• Valid CA Driver's License required; ability to travel 15–25 miles regularly.

Responsibilities

• Provide physical therapy services within the plan of care, coordinating with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report changes to the physician or nursing leadership.
• Maintain accurate clinical records using company-provided device and comply with policies.
