Job summary

Elara Caring is seeking a passionate Physical Therapist Assistant to join our home health team in Indiana. You will deliver PT services under the direction of a supervising Physical Therapist, helping patients regain mobility and independence in their own homes.

You will educate patients and families on home exercise programs, monitor progress, and ensure safe use of assistive devices. Travel between patient homes is required, with a focus on compassionate, high-quality care.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• Graduate of an accredited physical therapy assistant program approved by the American Therapy Association.
• 6+ months experience as a Physical Therapist Assistant is preferred.

Responsibilities

• Provide physical therapy services according to the treatment plan established by the Physical Therapist.
• Help patients work toward therapy goals that improve mobility, function, and independence.
• Monitor patient progress and promptly report changes in condition or concerns to the supervising Physical Therapist.
• Educate patients and caregivers on home therapy programs and techniques to support continued progress.
• Teach patients and families how to safely use assistive devices, including canes, walkers, wheelchairs, prosthetics, and orthotics.
• Deliver safe, high-quality therapy services in accordance with physician orders, clinical standards, and state practice guidelines
