Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Blue Hill, NE. Part Time or PRN role focused on delivering therapy services under PT direction to residents.

Responsibilities include screenings, executing treatment plans, and educating residents and staff. Requires an Associate's degree in Physical Therapy and active PTA licensure in state(s) of practice, plus ongoing licensure maintenance.

Qualifications

• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Carry out individual and group treatments per the plan, including mobility and strength work.
• Monitor residents’ response to intervention and consult with the PT for modifications.
• Assist in establishing resident schedules in coordination with the PT and team.
• Train residents, family and staff in use of equipment.
• Complete compliant and medically necessary documentation in CMS-compliant EMR.
