Job summary

Stern Therapy Consultants seeks a licensed Physical Therapist (PT) for a PRN role at Eastern Star Home and Campus, Oriskany, NY. You will evaluate residents, create plans of care, and deliver targeted interventions to improve mobility and independence.

Qualifications

• Active New York Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• New graduate Physical Therapists are welcome to apply.
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress throughout treatment.
• Modify treatment plans based on clinical needs and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs, nursing staff, and other rehabilitation professionals.

Physical Therapist (PT) – PRN Eastern Star Home and Campus | Oriskany, NY $45–$50/hour | PRN We are seeking a Physical Therapist (PT) for a PRN position at Eastern Star Home and Campus. What You'll Do
