Continuum Therapy Partners in Zumbrota, MN is seeking a Physical Therapist to join our team. A Doctor of Physical Therapy (DPT) degree and an active, unencumbered state license are required, with NPTE completion as part of the credentialing process.

The role may be PRN, offering flexibility to support our growing network of facilities. Applicants with about one year of PT experience will be preferred, and the successful candidate will evaluate and implement individualized treatment plans,

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
