Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our PRN team in Port Washington, Wisconsin. We value therapists who can work daytime, coordinate care with a supportive multidisciplinary team, and commit to patient-centered outcomes.

You’ll develop and implement individualized treatment plans, document progress, and educate patients and families on home programs. A DPT and active Wisconsin license are required, with NPTE completed, and at least 1 year of PT experience preferred.

Qualifications

• DPT degree is required.
• Current, unencumbered Wisconsin PT license; NPTE is required to obtain licensure.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements treatment plans in coordination with other disciplines.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
