Job summary

Continuum Therapy Partners is seeking a Physical Therapist for PRN daytime coverage at Oakwood Health Services in Altoona, WI. You will work as part of a supportive therapy team, designing and implementing individualized treatment plans alongside other disciplines.

The role requires a Doctor of Physical Therapy, an active Wisconsin PT license (NPTE), and about 1 year of prior PT experience preferred. Flexible scheduling with patient-focused care is offered.

Qualifications

• Doctor of Physical Therapy degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
