Job summary

Continuum Therapy Partners is seeking a Physical Therapist for a PRN daytime role in Altoona, WI, working with Oakwood Health Services. You will evaluate, develop, and implement individualized treatment plans, document progress, and educate patients and families on home exercise programs and injury prevention.

The position requires a DPT, WI PT license, and NPTE clearance, with about 1 year of experience preferred.

Qualifications

• Doctor of Physical Therapy (DPT) required.
• Current, unencumbered Wisconsin Physical Therapy License; NPTE required.
• 1 year of experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
