Job summary

Continuum Therapy Partners, LLC seeks a Physical Therapist for PRN scheduling at Suwannee Valley Nursing Center in Jasper, FL. You’ll join a team led by experienced therapists who value high-quality care and flexible hours.

The role requires a DPT, an active Florida license, NPTE compliance, and up to 1 year of PT experience. You’ll evaluate, plan, document, and educate patients and families while maintaining independent scheduling.

Qualifications

• DPT degree required.
• Current, unencumbered Florida PT license; NPTE required.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans.
• Documents patient progress and adjusts plans based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
