Job summary

Continuum Therapy Partners is seeking a Physical Therapist for PRN work at LaBelle Health and Rehabilitation Center in LaBelle, FL. The role emphasizes clinical voice, flexibility, and mentorship within a supportive team.

Must have a DPT, active license, and NPTE pass; 1 year of experience is preferred. You will evaluate, plan, document progress, educate patients, and coordinate with other disciplines. Schedule is Daytime PRN.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
