Job summary

Continuum Therapy Partners, LLC in West Chester, OH is seeking a licensed Physical Therapist to join our therapist-led network. A DPT is required, Ohio license, NPTE, and at least 1 year of experience preferred; you will evaluate and design personalized treatment plans and coordinate with care teams.

This PRN daytime position offers weekday and weekend coverage at Heritagespring - West Chester OH, with flexible scheduling to support growth, professional autonomy, and patient-centered care.

Qualifications

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
