Job summary

Evolve Therapy Services in Pahokee, FL is seeking a compassionate Physical Therapist for in-house Skilled Nursing Facility assignments. The role involves delivering PT services to residents, performing evaluations, and developing treatment plans in partnership with the interdisciplinary team.

You will document and bill services per CMS requirements using NetHealth EMR, maintain licensure through ongoing CE, and contribute to high-quality resident care with competitive pay and growth

Qualifications

• BS in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and productivity standards.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Evaluate residents to obtain data necessary for treatment planning.
• Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Complete CMS-compliant documentation in NetHealth EMR system.
