Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. The role requires collaboration with a Physical Therapist to provide resident care and to implement treatment plans with emphasis on mobility, strength, balance, and functional activities.

You will contribute to screenings, plan modifications, and patient/staff education while maintaining compliance with CMS and licensing requirements.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning under the Physical Therapist's direction.
• Carry out treatment plans including strength, mobility, balance, gait, and modalities as outlined by the Physical Therapist.
• Train residents and staff in equipment use and safety; participate in education for residents and families.
