Job summary

Evolve Therapy Services, LLC in Olivia, MN is seeking a licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to deliver resident-centered care.

Responsibilities include screenings, contributing to evaluations, developing treatment plans, guiding both individual and group sessions, documenting in NetHealth EMR, and coordinating schedules with the care team.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in developing treatment plans with the Physical Therapist.
• Conduct individual and group treatments per the treatment plan.
• Document compliant and medically necessary data in NetHealth EMR.
• Assist in coordinating resident schedules with the care team.
