Job summary

Evolve Therapy Services, LLC seeks a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Olivia, MN. The role involves delivering therapy under the PT’s supervision and helping develop treatment plans.

Qualifications include an Associate’s degree in physical therapy and an active PTA license. We offer full-time, part-time, and PRN options with competitive pay and benefits.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including strength, mobility, gait, ROM and modalities.
• Monitor residents’ response to intervention and modify as indicated.
