Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. This full-time position supports the Physical Therapist, conducts screenings, and helps develop and implement treatment plans to ensure high-quality resident care.

Responsibilities include patient evaluation, coordination with the care team, and documentation in NetHealth per CMS standards.

Qualifications

• Assoc. degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluating resident progress and updating treatment plans.
• Develop and modify individual resident schedules with the PT and team.
• Document care in NetHealth EMR per CMS expectations.
• Educate residents, families and staff on therapy activities and equipment.

Education
