Job summary

Evolve Therapy Services in Olivia, MN is seeking a Physical Therapy Assistant to join our in-house Skilled Nursing Facility team. You will work under the supervision of a Physical Therapist to deliver focused resident care and progress toward mobility and functional goals.

This full-time role includes a $5,000 sign-on bonus and tuition reimbursement, with options for Part Time or PRN schedules. The position emphasizes quality care, ongoing development, and collaborative teamwork.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying targets with short-term and long-term goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan including mobility, gait, range of motion, and modalities.
• Assist in selecting PT techniques/media and sequencing activities; adapt as indicated by resident performance.
• Monitor residents’ response to intervention and consult with the Physical Therapist if modification is needed.
• Assist in establishing and modifying individual resident schedules with the PT and team.
• Adhere to department and resident schedule and adjust for treatment regimens.
• Train resident, family and staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Report to work on time and coordinate schedule for productivity and efficiency.
• Assist with Quality Assurance Program as required.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
