Telos Health Systems in Charlotte, NC seeks a Psychotherapist (LCMHC, LMFT, or Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. The position offers flexible part-time or full-time schedules with a day-one impact and collaboration across multiple NC locations.

The role provides strong administrative and leadership support, clinical autonomy within a collaborative team, and a comprehensive benefits

Qualifications

• Licensed in-state as LCMHC, LMFT, or Psychologist (PsyD/Ph.D.).
• Master's or Doctoral degree in a relevant mental health field.
• Active, unrestricted license in the state of practice.
• Ability to work independently while collaborating within a clinical team.
• Strong communication and clinical documentation skills.

Responsabilités

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
