Job summary

Lakeside Behavioral Health System is seeking a Triage and Assessment Counselor/Nurse to perform risk assessments and crisis intervention, both by phone and in person, and to determine appropriate care levels. You will communicate with referral sources, families, patients and unit staff with empathy and urgency.

Qualifications

• Must hold Tennessee RN licensure and board eligibility.
• Experience in psychiatric crisis intervention preferred.
• Strong documentation and communication skills.
• Ability to assess risk and determine level of care.

Responsibilities

• Provide telephonic and in-person risk assessment and crisis intervention.
• Identify appropriate level of care and coordinate with referral sources, families, patients and staff.
• Document assessments accurately and in a timely manner.
• Demonstrate compassion and urgency in crisis situations.
