Job summary

Connections Health Solutions is seeking a Registered Nurse to assess individuals across adult and youth populations, document triage and health information, and contribute to crisis intervention and education. The role emphasizes safe patient handling, medication administration, and care transitions in a fast-paced behavioral health setting.

The successful candidate will hold an accredited nursing degree, an active RN license, and experience in crisis care; certification in crisis intervention

Qualifications

• Graduated from an accredited Nursing program.
• Unrestricted active RN license through PA State Board or multi-state licensure.
• Knowledge of Crisis Intervention and de-escalation techniques.

Responsibilities

• Nursing Assessment and Documentation: Complete detailed nursing assessments for adults and youth, document triage, medical/behavioral health, and medications.
• Treatment Planning: Plan individual treatment as part of the multidisciplinary team.
• Education and Health Literacy: Educate individuals and families on psychiatric diagnoses, treatments, and health literacy.
• Crisis Intervention: Intervene with verbal de-escalation and safety planning.
• Seclusion/Restraint Procedures: Knowledge of standards; document and debrief as required.
• Medication Administration and Documentation: Transcribe orders, administer meds, perform chart checks and wound care as ordered.
• Transitions in Care: Conduct nurse-to-nurse handoffs ensuring continuity.
• Additional Duties: Support overall care in adult and youth settings.
