Job summary

Heritage Oaks Hospital, part of the UHS family, seeks a Social Worker to perform biopsychosocial assessments, develop treatment plans, and prepare discharge plans including referrals for continued treatment and housing when appropriate. Qualifications

• Master’s Degree in Social Work or Marriage and Family Therapy required.
• Licensed or license-eligible with the California Board of Behavioral Sciences.
• Must have strong clinical assessment skills and crisis intervention familiarity.

Responsibilities

• Conduct psychosocial, educational, developmental, and behavioral assessments of patient/family systems.
• Document assessment updates when patient conditions change or on readmission within 30 days.
• Consult with families on emotional, social, and financial aspects of care.
• Provide group-based interventions and crisis interventions to support emotional health.

Education

Master’s Degree in Social Work or Marriage and Family Therapy

Licensed or license-eligible (CA Board of Behavioral Sciences)
