Job summary

Ochsner Health seeks a licensed social worker to conduct bio-psychosocial assessments, coordinate discharge plans, and connect patients and families with community resources. You will maintain records and communicate with the medical team while addressing emotional, social, cultural, and spiritual needs.

Requirements include a Master’s in Social Work (CSWE-accredited), certification as a CSW or RSW (LMSW preferred or to schedule LMSW within 6 months).

Qualifications

• Master's degree in Social Work (MSW) from CSWE-accredited program.
• CSW or RSW certifications (LABSWE verification).
• LMSW license preferred or must schedule LMSW exam within 6 months of employment.
• Two years experience in a medical setting preferred.

Responsibilities

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and communicates critical information to the medical team.
• Maintains professional and technical knowledge through continuing education and development.
• Participates in on-call rotation.
• Adapts communication to patient population while respecting privacy.
