Zusammenfassung

Heritage Oaks Hospital, part of the Universal Health Services family, seeks a Social Worker to provide biopsychosocial assessments, develop treatment plans, and support discharges. The role includes crisis interventions and group-based therapies for adolescents, adults, and seniors within a fully accredited 125-bed psychiatric setting in Northern Sacramento.

Minimum one year in a psychiatric facility and CA license eligibility are required.

Qualifikationen

• Master’s Degree in Social Work or Marriage and Family Therapy; license-eligible in CA.
• Minimum 1 year experience in a psychiatric health care facility with family and group therapies.
• Handle With Care training within 30 days; CPR certification required.
• May be required to work flexible hours and overtime.

Aufgaben

• Assess and identify psychosocial, educational, developmental and behavioral functioning of the patient/family system.
• Document assessment updates when there are significant changes in patient’s condition or readmission in the last 30 days.
• Consult with families on emotional, social and financial aspects of care.
• Develop a safe discharge plan including referrals for continued treatment and housing when appropriate.
