Job summary

Elara Caring is hiring an Registered Nurse to join our home health team in Massachusetts. You will assess patients, develop and update care plans, and provide ongoing evaluations during visits. The role includes supervising aides and coordinating with physicians to ensure quality care.

Requirements include an active RN license and 1+ year of clinical nursing experience; home health experience is preferred. A $5,000 sign-on bonus is offered along with comprehensive benefits and a base salary of

• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

Compensation

$5,000 SIGN ON BONUS

The base salary for this position is $85,000 to $90,000 annually based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity. * Benefit eligibility can vary by position and shift status.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
