Enjoy not working long facility shifts!****Help keep clients home where they most want to be!**As a **Registered Nurse Behavioral Health**, you'll contribute to our success in the following ways:* Assess patients' physical, emotional, social, and environmental needs.* Manage a caseload and develop, coordinate, and update individualized care plans.* Complete initial assessments and provide ongoing evaluations during patient visits.* Monitor patient progress and communicate changes with physicians, supervisors, and the care team.* Update care plans based on patients' changing needs.* Educate patients, families, and caregivers about the plan of care and behavioral health support.* Teach behavioral health care and symptom management to staff in partner care settings.* Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.* Report safety concerns or other issues to supervisors as needed.* Provide compassionate end-of-life support and complete bereavement assessments.* Current State license as a Registered Nurse.* Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.* 1+ year of experience as a nurse in a clinical care setting.* Home Health experience is preferred, but not required.* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.* Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance* 401(K) with employer match* Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost* Paid time off, paid holidays, family, and pet bereavement* Fleet vehicle program (restrictions apply) or mileage reimbursement.Benefit eligibility can vary by position and shift status.### The salary range for this position is $83,000 to $101,000 annually, based on the company's good faith estimate at the time of posting.
