Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health for PRN/Part-time visits in Norwich, New London, and Groton, CT. You’ll support patients in their homes with a focus on behavioral health, coordinating with an interdisciplinary team and ensuring high-quality, compassionate care.

You’ll conduct assessments, plan and implement care, educate families, and document findings. Travel up to 50% within branch coverage is required; vehicle and current RN license are essential.

Qualifications

• Current, unrestricted state RN license
• 1+ years of clinical nursing experience
• Ability to lift up to 50 pounds and travel 50%
• Must have dependable vehicle, valid driver’s license, and current auto insurance
• Willing to travel within branch coverage area

Responsibilities

• Assess patient needs and plan care per Interdisciplinary Plan of Care
• Perform initial evaluation and follow-up visits
• Communicate significant findings to supervisor and team
• Counsel and include patient and family in care plan
• Document care, issues, and outcomes
• Provide bereavement support as needed

Makes follow up visits according to patient, family, and facility needs.* Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.* Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.* Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.* Teaches behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).* Initiates, supervises, and documents observance of the behavioral health aide and LPN delivery of care per agency policy.* Informs supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.* Provides appropriate support at time of death and perform bereavement assessment.What is Required?* ### High School Diploma or GED* Current, unrestricted state RN licensee* 1+ years of experience in a clinical care setting as a nurse* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Able and willing to travel within branch/office coverage area.* Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.* Must be able and willing to travel 50%* Associates Degree or Bachelors Degree in Nursing is preferred* Previous Home Health or Hospice Experience is preferred* Previous experience with Homecare HomeBase is preferredYou will report to the Clinical Manager or Clinical Supervisor***Equal Employment Opportunity**: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
