Elara Caring is seeking an RN for Behavioral Health visits in Home Health, full-time. The role covers multiple territories including Amherst, Chicopee, Holyoke, and surrounding areas, with a $5,000 sign-on bonus.

You will assess, plan, implement, and monitor patient care while coordinating with physicians and the care team. The base salary ranges from $85,000 to $90,000 annually, with comprehensive benefits and opportunities for professional development.

RN Psych Visits Home Health FT

• Assess patients' physical, emotional, social, and environmental needs.


• Manage a caseload and develop, coordinate, and update individualized care plans.


• Complete initial assessments and provide ongoing evaluations during patient visits.


• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.


• Update care plans based on patients' changing needs.


• Educate patients, families, and caregivers about the plan of care and behavioral health support.


• Teach behavioral health care and symptom management to staff in partner care settings.


• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.


• Report safety concerns or other issues to supervisors as needed.


• Provide compassionate end-of-life support and complete bereavement assessments.

What is Required?

• Current State license as a Registered Nurse.


• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.


• 1+ year of experience as a nurse in a clinical care setting.


• Home Health experience is preferred, but not required.


• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment


• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $85,000 to $90,000 annually based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
