Job summary

ViaQuest is seeking a dedicated licensed practical nurse to join our psychiatric and behavioral health care team in the Dayton, OH area. You will perform nursing assessments, coordinate care, educate clients and caregivers, and support care management across long-term care facilities. Travel within the assigned service area is required.

Qualifications

• Graduate of an accredited nursing program.
• LPN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• CPR certification or course can be provided during training.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities.
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs
