Job summary

ViaQuest Psychiatric & Behavioral Solutions provides individualized care to clients in long-term care settings, covering dementia, anxiety, depression and more. You will perform nursing assessments, educate caregivers, and coordinate care with physicians, pharmacies and program staff across service locations.

The role requires graduation from an accredited nursing program and an active LPN or RN license, with travel across the Worthington and Canal Winchester service areas.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably experience in LTC facilities and Behavioral Health.
• CPR or Course can be provided during training.

Responsibilities

• Address the mental, behavioral and physical health needs of clients weekly/biweekly/monthly.
• Complete in person nursing assessments and medication/chart reviews in LTC facilities.
• Educate caregivers and clients on prescribed medications and monitor side effects.
• Care management including coordination of community services and communication with the treatment team.

Responsibilities may include:

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May need to cover more than one facility depending on company needs

Requirements for this position include:
