Job summary

Five Keys Housing coordinates comprehensive care for guests in interim housing, developing individualized plans and connecting them with housing, health, and social services. The role emphasizes trauma-informed, culturally sensitive support and close collaboration with partner agencies to ensure successful transitions to permanent housing.

Ideal candidates have two+ years in care coordination, strong HMIS, and excellent communication skills, with a commitment to serving unhoused populations

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling or related field, or equivalent combo of education/experience.
• Minimum two years of care coordination/case management experience.
• Minimum two years of paid work supporting unhoused populations with multiple disabilities.

Responsibilities

• Conduct thorough assessments of guests' needs, strengths, and challenges.
• Develop individualized care plans with guests and partners.
• Connect guests to housing, healthcare, mental health, and social supports.
• Advocate for guests' rights and needs with agencies and community orgs.
• Provide crisis intervention and follow-up after housing placement.
