Crestwood Terrace in Crestwood, IL is seeking a dedicated Psychiatric Rehabilitation Services Coordinator to deliver group and individual psychiatric rehabilitative and case management services for residents with complex mental health needs. You will work with an interdisciplinary team to tailor therapies and support long‑term stabilization.

Requirements include a Bachelor's degree in a human services field and at least 1 year of supervised mental health experience; PCC familiarity is preferred.

Qualificações

• Bachelor’s Degree in a human services field with at least 1 year of supervised mental health or human services experience.
• Experience in a licensed nursing home or intermediate care facility preferred.
• Ability to pass pre-employment physical and background check.

It’s so much more than what your current Psych/Social team is doing now!What you will do in this role: The Psychiatric Rehabilitation Services Coordinator (PRSC) provides group and individual psychiatric rehabilitative and case management services to adults with a history of multiple psychiatric hospitalizations and in need of long-term care stabilization.
