Job summary

College Medical Center in Hawthorne, CA is seeking a full-time AMFT/ASW Therapist for our outpatient PHP program. The therapist will run groups and provide some case management services within a growing program.

Responsibilities include initial assessments, treatment planning, progress notes within 24 hours, and discharge summaries. Applicants must hold an MFT, LCSW, or Psychologist license, or have an intern number with the Board of Behavioral Services or a Psych Assistant number.

Qualifications

  • Must be licensed MFT, LCSW, or Psychologist or have intern number registered with the Board of Behavioral Services or a Psych. Assistant number registered with the Board of Psychology.
  • Prefer 2 years of clinical experience with seriously mentally ill, developmentally disabled, or substance abuse clients.
  • Knowledge of psychiatric diagnosis, terminology, and medical record charting basics.
  • Good group facilitation skills and experience.
  • Must have a BLS for Healthcare Provider card; de-escalation training within 30 days of hire.
  • Facilitates group treatment modalities within scope of practice as assigned by the Program.
  • Conducts initial assessment of new patients and develops initial treatment plan.
  • Develops master treatment plans as prescribed by program policies and procedures.

Job Summary

CMC PHP is looking for a full-time AMFT/ASW Therapist. The AMFT/ASW Therapist will run groups and provide some, but minimal Case Management services for the program. This is an opportunity for a Therapist to be part of a growing Outpatient PHP Program.

  • Accurately enters progress notes within 24 hours of service or the next business day; progress notes are accurate, refers to problems identified on treatment plan. Documents incidents or unusual occurrences at the time it happens.
  • Completes discharge summaries for all assigned patients in a timely manner as prescribed
  • Effectively demonstrates competency working with the age specific needs of adult patients, ages 18+, and completes Psychosocial assessments conforming to the guidelines set forth in the Procedures for Psychosocial Assessment.
  • Assesses the needs of adult patients, ages 18+ and intervenes to facilitate movement to next stage.

Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CHLB/Glendora Hospital roles, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CHLB/Glendora Hospital in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
