Job summary

College Medical Center in Hawthorne, CA is seeking a full-time AMFT/ASW Therapist for our outpatient PHP program. The therapist will run groups and provide some case management services within a growing program.

Responsibilities include initial assessments, treatment planning, progress notes within 24 hours, and discharge summaries. Applicants must hold an MFT, LCSW, or Psychologist license, or have an intern number with the Board of Behavioral Services or a Psych Assistant number.

Qualifications

• Must be licensed MFT, LCSW, or Psychologist or have intern number registered with the Board of Behavioral Services or a Psych. Assistant number registered with the Board of Psychology.
• Prefer 2 years of clinical experience with seriously mentally ill, developmentally disabled, or substance abuse clients.
• Knowledge of psychiatric diagnosis, terminology, and medical record charting basics.
• Good group facilitation skills and experience.
• Must have a BLS for Healthcare Provider card; de-escalation training within 30 days of hire.
• Facilitates group treatment modalities within scope of practice as assigned by the Program.
• Conducts initial assessment of new patients and develops initial treatment plan.
• Develops master treatment plans as prescribed by program policies and procedures.

Responsibilities

• Run groups and provide some, but minimal Case Management services for the program.
• Develops master treatment plans, as prescribed by program policies and procedures, using data from all assessments and input from treatment team.
• Accurately enters progress notes within 24 hours of service or the next business day; notes refer to problems identified on the treatment plan.
• Completes discharge summaries for all assigned patients in a timely manner as prescribed.
• Assesses the needs of adult patients (18+), intervenes to facilitate movement to the next stage.
