Job summary

Community Care is hiring a Program Supervisor for the Family Care Program in Waukesha County. This full-time role leads the Case Management team, oversees day-to-day operations, and collaborates with leaders to ensure high-quality services for elders and adults with disabilities living independently in the community.

The position involves local travel, requires a Bachelor's in Nursing, Psychology, Social Work or Human Services, and may include licensure consideration.

Qualifications

• Bachelor's degree in Nursing, Psychology, Social Work or Human Services required or equivalent.
• Minimum 3 years experience providing case management for older adults, IDD. Leadership experience preferred.

Responsibilities

• Provide leadership and mentoring for the Case Management team.
• Directly engage in Case Management Activities.
• Coordinate and supervise day-to-day operations of the Family Care Program in collaboration with other leaders.
• Routinely travel locally with a valid driver's license and required insurance.
