Job summary

Community Care is seeking a Case Manager to join our mission-driven team. The role emphasizes community case management, collaboration with an RN Case Manager, and supporting elderly individuals and adults with disabilities to live as independently as possible in the community.

The position is located in Ozaukee County, with a Monday–Friday schedule and a $5,000 sign-on bonus. We offer competitive benefits and a strong focus on compassionate, personalized care for Wisconsin residents.

Qualifications

• Bachelor’s degree in a Human Services or closely related area required.
• Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities; may substitute for a non-human services degree.
• Eligibility for long-term care functional screener certification required.
• Travel: Regular local travel with own vehicle, valid license and insurance.

Responsibilities

Case Manager Job Requirements

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule

• Monday to Friday, 8:00 AM-4:30 PM
