Job summary

Horizon Health and Wellness, Inc. in Tempe, AZ, is seeking a Program Supervisor to oversee the provision of habilitation services and to supervise program staff, ensuring alignment with agency goals and regulatory requirements. The role requires a HS diploma with 1 year of supervisory experience or a Bachelor degree with supervisory exposure.

Bilingual English/Spanish or English/Sign Language is preferred. A valid driver’s license and fingerprint clearance are needed, and the candidate may work

Qualifications

• HS diploma or GED with 1 year supervisory experience required, or a Bachelor degree with some supervisory experience preferred.
• Typically supervises 2–8 FTE.
• Must be 21 years or older and have a valid driver's license with acceptable driving record.

Responsibilities

• Provide programmatic and staff supervisory responsibilities for habilitation services.
• Serve as liaison for the agency, direct support professionals, the community, families and participants in the program.
• Develop and maintain required documentation, ensure timely progress notes and billing.
• Maintain confidentiality and uphold the rights, dignity and privacy of persons assisted.
