Horizon Health and Wellness seeks a Direct Support Professional to provide care and support for individuals with intellectual and developmental disabilities in Tempe, AZ. The role emphasizes community integration, safety, and well-being within housing facilities.

The position requires a 21+ age, a valid driver’s license, and ability to manage required paperwork and documentation. Certifications training provided; fingerprint clearance and background checks required.

Qualifications

• High School diploma or GED required.
• 21 years of age or older.
• Valid driver's license with acceptable driving record and reliable transportation.
• Ability to prepare required paperwork within contractually required timelines.
• Typing ability and familiarity with computers and electronic records.
• Ability to complete required certifications and trainings (First Aid, CPR, Article 9, Medication, and Prevention/Support, etc.).
• Ability to work independently with minimal supervision.
• Strong communication skills with participants and coworkers.
• Must obtain fingerprint clearance card and pass background checks.
• Willingness to work nights, weekends, holidays, and varied schedules.

Responsibilities

• Provides care services to individuals with intellectual and developmental disabilities.
• Facilitates community integration and inclusion for participants.
• Develops opportunities for participation in community activities while ensuring safety and wellbeing in the residential setting.
