Job summary

Saint Joseph’s Center in Dunmore, PA is seeking a Program Specialist for Community Living Arrangements. You will oversee residential sites, supervise staff, and develop Individual Support Plans to help individuals achieve independence and life goals.

You will coordinate schedules, conduct site visits, and ensure timely ISP documentation while collaborating with families and providers. Travel between program locations and on-call duties may be required as part of the role.

Qualifications

• Associates degree or 60 college credits minimum, or a Bachelor’s degree with relevant experience.
• Minimum 4 years’ experience working with individuals with disabilities; supervisory experience preferred.
• Strong communication, organization, and leadership skills.
• Valid PA Driver’s License and ability to travel between sites.
• Ability to sit, stand, and walk throughout the day; ability to lift light items.
• Willingness to travel regularly between program locations.

Responsibilities

• Supervise and support residential staff, including training and performance oversight
• Develop, implement, and monitor Individual Support Plans (ISPs)
• Attend ISP meetings and ensure assessments, outcomes, and documentation are completed on time
• Conduct regular site visits and ensure quality care across all assigned homes
• Coordinate staff schedules, review time records, and assist with hiring
• Respond to on-call emergencies and complete required reports and investigations
• Ensure compliance with 6400 regulations and program policies
• Collaborate with families, support coordinators, and outside providers
• Support community-based activities and monitor individual progress
• Lead staff meetings, trainings, and ongoing development
