Qualifications

Responsibilities

  • Oversees referral, intake, admission, and program navigation processes from initial referral through program placement.
  • Completes and oversees clinical intakes, assessments, and individual service plans while ensuring timely access to services.
  • Coordinates with referral sources, clients, and interdisciplinary teams to support timely admissions and appropriate placement decisions.
  • Oversees program navigation services and supports clients in accessing behavioral health, physical health, benefits, housing, and community resources.
  • Coordinates client move-in activities and ensures eligibility requirements, benefits, and support services are established prior to admission.
  • Maintains accurate, professional, and timely client records in compliance with organizational, regulatory, contractual, and accreditation standards.
  • Reviews operational, production, business intelligence, and unbilled reports to monitor program performance and identify improvement opportunities.
  • Monitors documentation quality, compliance requirements, and program outcomes while supporting continuous quality improvement initiatives.

Position Summary

The Program Supervisor, WSH Program Navigation, provides leadership, supervision, and operational oversight to Weldon Supported Housing (WSH) Program Navigation staff, including Intake Coordinators and Integrated Health Coaches. This role is responsible for overseeing referrals, intakes, assessments, care coordination, and connections to community resources. The position is essential in ensuring effective coordination of client services across supportive housing, behavioral health, physical health, eligibility and benefits, and community-based programs. This position reports to the WSH Clinical Director and works collaboratively with organizational leadership, clinical teams, internal departments, and community partners to promote continuity of care, regulatory compliance, and high-quality service delivery.

Duties May Include

Qualifications
  • Education – Bachelor's degree in a medical, behavioral health, or related field required; Associate degree with relevant experience may be considered. Master's degree in a behavioral health field and behavioral health licensure strongly preferred.
  • Years experience – Minimum of two (2) years of experience in the substance use, behavioral health, or related field.
  • Training preferred/required – Supervisory, behavioral health, care coordination, and supportive housing experience preferred.
  • Skills preferred/required –
    • Strong leadership and staff supervision skills.
    • Excellent communication and interpersonal skills.
    • Strong clinical assessment and care coordination capabilities.
    • Knowledge of behavioral health, Serious Mental Illness (SMI), Substance Use Disorders (SUD), and integrated healthcare services.
    • Understanding of supportive housing models and the needs of individuals experiencing housing instability.
    • Proficiency with electronic health records, reporting systems, and documentation standards.
  • Must have a valid Arizona driver's license, be at least 21 years of age, have a minimum of three years of driving experience, and meet Terros Health driving requirements.
  • Must successfully complete TB screening and criminal background requirements.
  • Must possess or obtain a valid Arizona Level 1 Fingerprint Clearance Card within seven working days of assuming the role.
