Job summary

Terros Health seeks a Case Manager at the Desert Cove Recovery Health Center in Phoenix, AZ. You will join an interdisciplinary clinical team serving individuals with Serious Mental Illness or General Mental Health diagnoses under Court-Ordered Treatment.

Full-Time Employed: 40 hours/week, M-F 8:30a - 5:30p, located at 10844 N. 23rd Ave. Suite 200, Phoenix, AZ 85029. Benefits include 401K, PTO, and ongoing training.

Qualifications

• GED/High school diploma with behavioral health experience or relevant degree
• Associates degree in medical or behavioral health may be considered
• Bachelor's degree (not in behavioral health) with 1 year of behavioral health experience acceptable
• Master's or higher in behavioral health strongly preferred
• Proficiency in MS Word, Excel, Outlook; EMR experience preferred

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services
• Completes assessments, crisis plans and individualized service plans

Case Manager Position

Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Desert Cove Recovery Health Center.

Duties

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
• Completes assessments, crisis plans and individualized service plans
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented
• Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
• Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations
• Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure

• An associatesdegree in the field of medical or behavioral,or
• A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience,or
• A bachelor's degree in the field of behavioral,or
• A master's degree or higher in the field of behavioral health
• Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
• Valid AZ Driver License.Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
• Must have a valid Arizona Fingerprint Clearance card (Level 1)orapply for an Arizona Fingerprint Clearance card within 7 working days of employment
• Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
• Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
• Basic understanding of the DSM-V preferred
• Certified Case Manager strongly preferred
• Must pass background check, TB test and other pre-employment screening.
