Job summary

Community Services Group is seeking a Program Specialist to coordinate assessments and develop Individual Support Plans (ISPs) for individuals with intellectual and developmental disabilities (IDD). The role focuses on promoting independence across daily living, communication, and adaptive skills, reporting to the Program Director.

This full-time, 40-hour-per-week position runs Monday through Friday from 8 AM to 4 PM, with flexible hours as needed.

Qualifications

• Master's degree or higher with 1 year work experience directly with individuals with an intellectual disability or autism.
• Bachelor's degree with 2 years work experience directly with individuals with an intellectual disability or autism.
• Associate's degree or 60 credit hours with 4 years work experience directly with individuals with an intellectual disability or autism.

Responsibilities

• Provides training to employees including new employee on the job training and orientation.
• Coordinates and completes initial and on-going assessments of individuals.
• Coordinates and develops, reviews, and ensures implementation of individual support plans.
• Works with behavior specialists or therapists to ensure plans are implemented and progress documented.
• Develops and ensures implementation of planned program activities.
• Coordinates and serves as team leader in multidisciplinary team meetings pertaining to the individual's service plan.
• Develops educational materials and provides assistance to employees related to Person-Centered Planning, ISPs, Outcomes and Self-Determination.
• Reports and enters incidents as outlined in the Incident Management Bulletin and policy.

Education

Master's degree or above + 1 year IDD/autism experience

Bachelor's degree + 2 years IDD/autism experience

Associate's degree or 60 credit hours + 4 years IDD/autism experience

Summary

The Program Specialist is responsible for the completion and coordination of the individual's assessment, for the development, review, update, and revision of the Individual Support Plan (ISP). You are responsible for the implementation of goal plans in the areas of independent functioning, physical development, communication skills, academic, adaptive behavior, community skills and daily living skills in order to promote greater independence. The Program Specialist reports directly to the Program Director.

This position is a part of our Adult Intellectual and/or Disabilities (IDD) Services.

Full-Time (40 hours per week) with Monday to Friday - 8AM to 4PM Hours. Must be able to work flexible hours to meet individual's and program's needs.

Wage Information

$25.00/hr. with increase possible based on relevant IDD experience.

Job Description

• Develops educational and training materials, presents educational materials on various aspects of programs and provides assistance to employees related to Person-Centered Planning, ISPs, Outcomes and Self-Determination.
• Reports and enters incidents as outlined in the Incident Management Bulletin and CSG's policy on Incident Management.

Qualifications

This position requires one of the following combinations of education and experience:

• A master's degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism
• A bachelor's degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism
• An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism.

Additional Requirements Include

• A valid driver's license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.
• Able to work flexible hours.
