Job summary

Community Services Group is seeking a Program Specialist to coordinate assessments and develop, review, and revise Individual Support Plans (ISPs) to promote greater independence for individuals with IDD. The role requires collaboration with a multidisciplinary team to implement goal plans across daily living, communication, and adaptive skills.

The position is full-time (40 hours/week) with Monday–Friday, 8 AM–4 PM; flexibility to meet individuals’ and program needs is required.

Qualifications

  • Master’s degree or higher with 1 year of IDD/autism-related experience
  • Bachelor’s degree with 2 years of IDD/autism-related experience
  • Associate’s degree (60 credits) with 4 years of IDD/autism-related experience

Responsibilities

  • Provides training to employees including on-the-job training and orientation.
  • Coordinates and completes initial and ongoing assessments of individuals.
  • Coordinates and develops, reviews, and ensures implementation of individual support plans.
  • Collaborates with behavior specialists or therapists to ensure plans progress and are documented.
  • Develops and implements planned program activities.
  • Leads multidisciplinary team meetings related to the individual’s service plan.
  • Develops educational materials and supports staff on Person-Centered Planning and ISPs.
  • Reports incidents according to policy and documents accordingly.

Summary

The Program Specialist is responsible for the completion and coordination of the individual’s assessment, for the development, review, update, and revision of the Individual Support Plan (ISP). You are responsible for the implementation of goal plans in the areas of independent functioning, physical development, communication skills, academic, adaptive behavior, community skills and daily living skills in order to promote greater independence. The Program Specialist reports directly to the Program Director.

This position is a part of our Adult Intellectual and/or Disabilities (IDD) Services.

Full-Time (40 hours per week) with Monday to Friday - 8AM to 4PM Hours. Must be able to work flexible hours to meet individual's and program's needs.

Wage Information

$25.00/hr. with increase possible based on relevant IDD experience.

Job Description
  • Provides training to employees which includes new employee on the job training and orientation.
  • Coordinates and completes initial and on-going assessments of individuals.
  • Coordinates and develops, reviews, and ensures implementation of individual support plans.
  • Works collaboratively with other professionals involved in the individual’s services, such as behavior specialists or therapists, to ensure plans are implemented and progress documented.
  • Develops and ensures implementation of planned program activities.
  • Coordinates and serves as team leader in multidisciplinary team meetings pertaining to the individual’s service plan.
  • Develops educational and training materials, presents educational materials on various aspects of programs and provides assistance to employees related to Person-Centered Planning, ISPs, Outcomes and Self-Determination.
  • Reports and enters incidents as outlined in the Incident Management Bulletin and CSG’s policy on Incident Management.
Qualifications

This position requires one of the following combinations of education and experience: A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism or A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism ; or An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism.

Additional Requirements Include
  • A valid driver’s license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.
