Job summary

Caring Hands & More LLC in Iowa City, IA is seeking an experienced Case Management Leader to oversee operations across our sites and coordinate care plans for individuals we serve. This role leads Case Management and Site Supervisors, develops the agency ISP, conducts reviews, and ensures compliance with rights and confidentiality while partnering with providers and the care team.

Qualifications

• Bachelor’s degree in social services or related field with 1 year of related experience, OR an associate’s degree with 3 years of related experience, OR a High School Diploma/GED with 5 years of related experience.
• Previous supervisory experience is required.

• Strong problem-solving abilities
• Effective verbal, written, and interpersonal skills
• Demonstrated ability to maintain professionalism and confidentiality with clients, staff members, agency leadership, and outside stakeholders
• Intermediate to advanced computer skills

Responsibilities

• Oversee and implement the individual’s plan of care.
• Develop the agency Individual Service Plan (ISP) that supports the goals and desires of the individuals we serve.
• Conduct annual and quarterly reviews to re-evaluate the supports provided while advocating for adjustments needed to the Person-Centered Service Plan (PCSP).
• Collaborate with the individual and their care team to review and evaluate any current rights restrictions in place or in need of implementation.
• Oversee operations for all assigned sites.
• Coach Site Supervisors with training, individual consultation and modeling desired outcomes.
• Provide constructive feedback through regular meetings and staff evaluations.
• Plan and coordinate services for the individuals we serve.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Case Management Role

• Serve as the point of contact for all other service providers.
• Collaborate with the Individuals we serve, Case Managers, MCOs, Guardians, natural supports, and other service providers.
• Oversee and implement the individual’s plan of care.
• Develop the agency Individual Service Plan (ISP) that supports the goals and desires of the individuals we serve.
• Conduct annual and quarterly reviews to re-evaluate the supports provided while advocating for adjustments needed to the Person-Centered Service Plan (PCSP).
• Collaborate with the individual and their care team to review and evaluate any current rights restrictions in place or in need of implementation.
• Conduct individual file audits to ensure all information needed is included in the individual’s file.
• Work with other Program Managers and the Director to evaluate program success, identify barriers to success and promote solutions to improve the services we provide to the individuals we serve.

Leadership Role

• Oversee operations for all assigned sites.
• Serve as a coach and mentor to Site Supervisors through training, individual consultation and modeling desired outcomes.
• Provide constructive feedback through regular meetings and staff evaluations.
• Work with Site Supervisors to address any house concerns, scheduling concerns or other barriers to their success or the success of the individuals we serve.
• Collaborate with Site Supervisors to plan and coordinate services for the individuals we serve.
• Oversee the Site Supervisor as they set appointments and arrange transportation for the individuals we serve.
• Actively participate in selection of new personnel and transfers.

Qualifications

To qualify, you need to be at least 21 years of age and meet one of the following conditions: Possess a bachelor’s degree in social services or related field of study and 1 year of related experience or an associate’s degree and a minimum of 3 years of directly related experience or a High School Diploma/GED and 5 years of directly related experience. Previous supervisory experience is required.
