• Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required

Responsibilities

• Provide initial and ongoing assessment and develop person-centered plans.
• Ensure services match the person-centered plan and are fiscally viable.
• Train staff on plans and conduct clinical supervision.
• Maintain records in the EHR/medical records.
• Respond to incidents following agency policies and procedures.
• Driving and travel may be required.
